Carnage is the hardest difficulty in Onimusha: Way of the Sword, unlocked after completing the main story (NG+ only). Bosses hit harder, faster, and have new attacks. Required for True Onimusha and Glutton for Punishment trophies.
1. What is Carnage Mode?
Carnage is the highest difficulty tier in Onimusha: Way of the Sword, sitting above Action and the unlockable mid-tier modes. It is only available after you have completed the main story at least once, which means it is exclusively a New Game Plus mode.
On Carnage, the game expects you to have already mastered the basics. The difficulty curve changes in three key ways:
- Bosses are stronger, faster, and have new attacks — every main story boss gains an enhanced moveset and shorter recovery windows.
- Action Prompts are disabled — you must learn attack timing without the on-screen hints you relied on in earlier playthroughs.
- Damage scaling — some bosses can 2-shot you at full health, especially during enhanced-state phases.
Carnage is required for two trophies: True Onimusha (clear the game on Carnage) and Glutton for Punishment (defeat every Boss Rematch opponent on Carnage).
You cannot pick Carnage on your first playthrough. Plan your first run on Action (or a balanced mid-tier) and reserve Carnage for NG+. Higher difficulties don't add new story content — they raise the skill ceiling and unlock specific trophies.
2. General Survival Tips
These tips come from PowerPyx's Carnage survival guidance. Apply them from the moment you re-enter Kyoto.
- Choose the Defense fighting style — it is more forgiving than offense-heavy styles and gives you access to stronger guard and parry tools. The added safety is critical when bosses 2-shot you.
- Turn OFF Action Prompts during your first playthrough — disabling prompts early forces you to learn attack timing from animation, which is mandatory on Carnage.
- Master Deflect — successful deflects deal stamina damage to bosses, which is the key to triggering Break Issen windows for burst damage.
- Don't button mash — bosses on Carnage punish greed ruthlessly. Commit to 2-3 hit combos, then reset to neutral.
- Avoid early damage spikes — taking too much damage too early in a boss fight can push them into an enhanced state sooner, drastically shortening your safe windows.
- Save your consumables — Amulets of Introspection, Sun-Dried Hozukis, and Stimulants are scarce. Stockpile during the run and deploy at critical phases.
If you're unsure which fighting style to mainline, pick Defense on Carnage. The damage reduction on successful blocks and the easier parry windows more than make up for the lower burst offense. You can supplement with Oni Gauntlet attacks when bosses are staggered.
3. Carnage-Specific Boss Mechanics
Every boss in Carnage gets a more aggressive moveset. Some gain entirely new attacks you won't have seen before.
- Enhanced state triggers sooner — bosses slip into their enhanced (rage / phase 2) state much earlier than on lower difficulties. Plan burst damage windows around this threshold.
- New attack patterns — several bosses gain completely new combos that don't appear on Action difficulty. Watch for telegraphs you haven't seen.
- Higher damage ceiling — some bosses can 2-shot you from full HP if you mistime a dodge. Always assume one mistake = full punish.
- Shorter recovery windows — bosses recover from whiffed attacks faster, punishing your counter-attack timing.

Stockpile these six consumables during your NG+ run. Use them only when a boss forces a critical moment.
Don't spend consumables on standard encounters on Carnage — every fight matters, but the difference between clearing and failing usually comes down to how many Heavies and Stimulants you bring to the major boss doors. Save three of each for the final boss.
7. Build for Minamoto no Yoshitsune (Final Boss)
The final boss on Carnage requires the entire recommended kit plus every consumable you've saved:
- Charm of Awakening (max)
- Charm of Oni Power (max)
- Fencer's Strength Charm (max)
- Default Hozuki Pouch (max)
- All 6 consumables listed above (Heavy Restorative, Amulet of Introspection, Stimulants, Sun-Dried Hozuki, Greater Defense Talisman, Greater Might Talisman)
Yoshitsune has 3 phases and enhanced attack patterns on Carnage. Don't deploy resources in phase 1 — bait out enhanced state, then burn the Stimulants + Greater Might Talisman to burst him down before phase 3 overwhelms you.
8. Boss Rematch on Carnage
After clearing the main story on Carnage, Boss Rematch becomes available with all bosses. Rematch runs reward you with bonus materials but are tuned for Carnage-level aggression.
To earn the Glutton for Punishment trophy, defeat every available Boss Rematch opponent on Carnage. Most can be brute-forced with the recommended build above; the outliers are Sasaki Ganryu (altered moveset, 2nd hardest in the game) and Shuten Doji (3 phases with Rage Mode).

Glutton for Punishment — Defeat every Boss Rematch opponent on Carnage.
